Social Prescribing

Social prescribing is a way for GPs, other health professionals and local agencies to refer people to a Link Worker. 

Link Workers give people time, focusing on what matters to them using a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Social prescribing works for a wide range of people, including those with one or more long-term conditions, who need support with their mental health, who are lonely or isolated or who have complex social needs which affect their wellbeing.

Heeley Trust’s link worker team includes; Maxine, Jenna, Ben, Katie, Jenny and Bev. They receive referrals from GP practices in the Heeley Plus Primary Care Network and from other local agencies. They also accept self referrals from those live locally.

  • Gleadless Medical Centre
  • Heeley Green Surgery
  • Sharrow Medical Centre
  • Sloan Medical Centre – Main site and Blackstock Road
  • The Mathews Practice – Asline Road and White Lane
  • Veritas Health Centre

To make an initial appointment with one of our Link Workers, you can be referred by your GP practice (if it is part of the Heeley PCN) or if you live locally in Heeley and the surrounding area by another agency such as Age UK. Alternatively, you can self-refer by calling Heeley Trust on 0114 399 1070.
